Step 2: Gather Medical Documentation
You’ll need records confirming your child’s diagnosis and care needs, such as tracheostomy, seizure disorder, or developmental delay.
We’ll request these directly from your physician, if needed.
Step 3: Get the Letter of Medical Necessity (LMN)
Your child’s doctor must fill out an LMN explaining:
The child’s condition
Why in-home care is needed
What level of care is recommended
We coordinate this step with your provider.
Step 4: We Submit Your Application to GAPP
